IMPORTANT INFORMATION
A prescription is not guaranteed with an appointment.
Whole Hearts Clinic does not prescribe ADHD medications unless at least one of the following is provided: a prior diagnosis documented in medical records sent or uploaded to our clinic, with a history of ADHD medication use as shown in the CURES report, or completed ADHD testing with results submitted to us.
Prescriptions will only be written and/or renewed during an appointment. Prescription medications require close monitoring by the provider to ensurethe safe continuation of the appropriate dose, frequency, and term of those medications. The provider will prescribe the appropriate number ofprescription refills to last until the next scheduled appointment.
It is the patient’s responsibility to schedule the next appointment in advance and with adequate time to receive a prescription renewal. Missedappointments and last-minute cancellations may result in the denial of renewals.
We cannot replace lost or stolen prescriptions for controlled substances. Patients must secure these medications carefully.
Patients may only have one provider who prescribes a controlled substance. Obtaining a controlled substance (or similar medication) from a source other than the Whole Hearts Clinic / Carasquero Nursing Inc. provider may result in denial of renewals.
